Founder of Personalized Care Coordination, LLC

Michelle's commitment to care coordination is rooted in both her nursing career and personal experience. When her mother faced a health crisis, Michelle witnessed firsthand how overwhelming and confusing navigating the healthcare system can be, particularly when someone is unwell and faced with numerous medical decisions, appointments, and care needs. This experience reinforced how difficult it can be for individuals and families to coordinate care and find clarity during an already stressful time. Families deserve knowledgeable guidance and a dedicated advocate. This experience helped inspire her to establish Personalized Care Coordination, LLC.


Michelle earned her Bachelor of Science in Nursing from the University of Rhode Island and has experience in both skilled nursing and primary care. She draws on her clinical knowledge and understanding of the healthcare system to help her clients and their families coordinate medical care, navigate complex needs, and ensure important concerns are communicated and addressed. Her approach is rooted in the belief that care should always be personal. By collaborating with clients, healthcare providers, families, and community resources, Michelle develops individualized care plans that respect each person's wishes, support quality of life, and promote the greatest level of independence possible.

Elder Care Management Services

Personalized Care Coordination, LLC provides support in the areas of advocacy, nursing assessment, care management, consultation, crisis intervention, education, referral services, and placement.

Advocacy

We will act as an advocate to promote maximum wellness and independence for your loved one.

Nursing Assessment

We will conduct a comprehensive assessment of our client's and their environments in an effort to help identify their strengths and their challenges. 

Care Management

Care Management includes implementation of the plan of care, regular visits to the client’s place of residence, accompaniment to major medical appointments and navigation through the health care system.

Consultation

We are available to answer questions for our clients and their family members regarding support, in home services and end of life planning.

Crisis Intervention

We strive to avoid a crisis, but should one arise, you can count on our team to be there for you or your loved one.

Education 

Knowledge is power. Our goal is to inform the client about their condition and empower them to make the appropriate decisions regarding care.

Referral Services

We will only refer to the best care providers in the area, such as homemaker companion agencies, elder law attorneys, geriatric physicians, etc.  This enables our client, and their family to then make informed decisions that best fit their needs.

Placement

We will assist in finding the best alternative living arrangement for our client should they wish to live in an assisted living facility or when the senior is no longer safe to live alone in their own home. 

The underlying goal of Personalized Care Coordination is to promote dignity and respect and make a positive difference in the lives of area seniors and their families.
