During the initial phone conversation, we will discuss the issues that are presenting the greatest challenges along with the possible ways we can help you or your loved one.

How We Can Help

In-home Consultation

An appointment will be made to come out to your home to meet with you.  We will assess current needs, address concerns and answer questions regarding care.

  • This includes

    • A review of all medical conditions and challenges
    • Obtaining a list of all current prescriptions and over the counter medications
    • Determining the client’s ability to live safely on their own
    • Evaluating the living space to see it if supports the client’s safety and well-being
    • Reviewing legal needs, such as wills, Advance Directives, and Powers of Attorney

How We Can Help

In-home Evaluation

Once the assessment is complete, a Plan of Care will be developed which may consist of:

  • This includes

    • A summary of all key medical diagnoses, medications and current treatment regimens
    • Referrals to doctors, specialists, and other medical professionals who can help address issues of immediate concern
    • Suggesting home modifications 
    • Make referrals to a home health or companion agency
    • Providing a list of resources to assist the family in gaining access to information to their specific areas of concern
    • Referrals to senior living communities both for short term respite or long term planning, if needed

How We Can Help

Monitor and Follow Up

We will work closely with you and your family to ensure that the care plan created is implemented and progressing toward the established goals.

  • This includes

    • The primary contact for the family and health care team
    • Attending medical, financial or legal appointments with clients and advocating on their behalf
    • Conducting regularly scheduled meetings with family members to monitor progress and address any new concerns
    • Medication management and education
    • Clinical oversite to assure appropriate care is in place
    • Responding in times of crisis
